Know someone who needs help at home, or someone already caring for a loved one who could be paid for it? Supports Coordinators, service coordinators, hospital discharge planners, community partners and families can all refer to Prestige Home Care. We follow up promptly on every referral.

Who can you refer?
Refer a client
Adults 21 and older who need help with daily activities to live safely at home: bathing, dressing, mobility and transfers, meals, light housekeeping, companionship and respite. We serve PA Medicaid Community HealthChoices participants (UPMC Community HealthChoices and AmeriHealth Caritas PA Community HealthChoices); ODP waiver participants 21 and older with intellectual disabilities or autism (Consolidated, Community Living and P/FDS waivers), with supports delivered according to each person's Individual Support Plan; and private pay clients.
Refer a family caregiver
Is someone already caring for a parent, relative or friend? Through PA Medicaid home care programs, adult children, other relatives and friends can often be hired and paid as that person's caregiver through Prestige. Spouses, legal guardians and powers of attorney can't be paid under Community HealthChoices, but we can help them find respite and other support. Eligibility rules apply, and we walk families through every step.
What referral partners can expect
A prompt response to every referral, a Registered Nurse on our team, trained and screened caregivers, and help with Medicaid paperwork from start to finish.

Non-skilled home care for adults 21+ who have physical, cognitive, or developmental limitations that require assistance with ADL or IADL needs to live safely at home, paid through PA Medicaid or private pay. We also provide ODP waiver services for individuals with intellectual disabilities and autism.
