How it works
Program Eligibility
Step 1 : Initial Contact & Assessment
Contact the PA Independent Enrollment Broker (IEB) or local County Assistance Office (CAO) to initiate the process. A Functional Eligibility Determination (assessment) is conducted to ensure the applicant needs a nursing home level of care.
Step 2: Medical Certification
A doctor must complete a Physician Certification Form (MA 51) to confirm the need for long-term services.
Step 3: Financial Eligibility Review
The CAO reviews income and assets (resource limits differ for individuals vs. couples). As of 2025, special rules (like MAWD) may allow those over the income limit to qualify.
Step 4: Service Plan Development
If medically and financially eligible, a service coordinator works with the applicant to develop an Individualized Service Plan (ISP) to determine needed services.
Step 5: Enrollment:
Upon approval, which generally occurs within 90 days of the application, the applicant is enrolled in the specific waiver program (e.g., Community HealthChoices).
